Some important transition points are going to school for the first time, going to middle and high school, graduation, moving, or getting married. One of these transitions is medical transition. This happens when you move from pediatric to adult health care. Medical transition is important because it helps your child take control of health care and be more self-determined. It also helps you understand the adult health care system before you get there. Medical transition provides you the opportunity to have health insurance in place when youth become adults. When making this transition it is important to start slow, start small, and start now. By starting slow your plans and progress through transition is manageable for you. Starting small allows you to do the simple things and plan for the bigger pieces

There are many components to caring for our health. Health care goes beyond the people who help to take care of us and into areas that may not seem so common to us. Merriam-Webster’s online dictionary defines “health care” as ‘the maintaining and restoration of health by the treatment and prevention of disease especially by trained and licensed professionals (as in medicine, dentistry, clinical psychology and public health)’. For parents and family members, it’s the thought of buying school clothes and supplies . . . or maybe wondering how they’ll do themselves on the first day of school watching their child enter the school doors or get on the bus for the first time . . . or being a strong part of getting their child’s IEP or 504 plan together.
