How and why is that? Well since this week’s edition focuses on small scale assisted living we will restrict this business model. Let’s just imagine operating an eight, (8) bed program. Finally you have reached capacity, every bed is full. Of course we know we are dealing with humans so there is really no constant. Some r others require a higher level of care or move simply due to a change in personal or family preference. It happens every day, all day long. “Project Admit-a Clearinghouse” Brings Peace of Mind to Families and Nursing Homes SERVICE DELIVERY NEWS April 2012 Bob Macdonald, dealing with his own serious limitations was overwhelmed with all of the legwork involved in trying to find a nursing home which would accept his wife’s admission. A fall added to her complex medical condition and she was on a 30-Day Notice of Involuntary Discharge from the non-Medicaid participating nursing home. The solution? Project-Admit. A care planning counselor jumped in, made contact with over 25 Medicaid participating homes and arranged for one to accept Mrs. Macdonald’s admission. The discharging nursing home was notified to supply all the required documentation and the matter proceeded expeditiously. Rita Mason’s situation was a bit more complicated. She was told her HMO would only pay a few more days or she needed $9000. 00 to keep her husband in the current nursing home. Arrangements for prompt discharge were needed. She spoke to an at

April 9, 2012 T his is the only on-line journal in the State of Michigan which specifically speaks to vacancies and program descriptions in assisted living. Where possible photos are included. There is also attention drawn to adult day health care, on-site physician care and other community based care initiatives. It is also a part of a wider effort to help nursing homes to interact with one another to advertise the availability of and need to transition patients to Medicaid reimbursed, long-term beds. Nursing home participation is governed by a simple subscription service discussed in one of this week’s feature articles. Our goal is to provide families, case managers, family caregivers, area agencies on aging staff along with hospital and nursing home discharge personnel a glimpse into what is available along the spectrum of community based and sometimes institutional care.
