There is also attention
drawn to adult day health care, on-site physician care
and other community based care initiatives.
It is also a part of a wider effort to help nursing homes
to interact with one another to advertise the availability
of and need to transition patients to Medicaid reimbursed, long-term beds and into assisted living communities, through Project Admit: A Clearinghouse.
Nursing

Our goal is to provide families, case managers, family
caregivers, area agencies on aging staff along with hospital and nursing home discharge personnel a glimpse
into what is available along the spectrum of community
